    What is closed nose surgery?

    Scarless rhinoplasty, often known as “closed” rhinoplasty, is a surgical method used to improve the appearance of the nose without the need for external incisions. A scarless rhinoplasty procedure can be used to make the following changes to the nose:

    • Narrowing a nose
    • Elevating a droopy nose
    • Augmenting the bridge of the nose
    • Reducing a dorsal hump on the bridge
    • Deprojecting a nose that has overprotruded (comes too far out of the face)
    • Refining a bulbous or boxy nasal tip


    Our surgeons can conduct revision rhinoplasty using the closed technique in some circumstances, but only in the hands of experts. The most significant benefit of scarless rhinoplasty is that there are no visible scars afterward. There is also less postoperative discomfort and edema and more predictable healing since there is less manipulation of nasal tissues, especially with a method that generally involves minimal or no skin dissection off the bridge of the nose or an atraumatic subperichondrial dissection.

    One of the most contentious issues among rhinoplasty surgeons is whether the procedure should be performed in a “closed” or “open” manner. Many surgeons who start their rhinoplasty careers performing closed rhinoplasties believe that the closed technique is the “elite” approach since it leaves no external scar and is technically more difficult to execute. On the other hand, surgeons who use the “open” approach mostly or exclusively would say that the scar is essentially insignificant over time because it goes away, and it allows for complete visualization and manipulation of the nose’s components.

    To begin, it should be noted that, while the closed technique is lauded since it leaves no visible scar, producing a nose that lacks the characteristics of an “ideal nose” in order to avoid a scar is futile. More often than not, this is what happens when surgeons use a closed approach. The normal outcome of a closed rhinoplasty is a nose that appears slightly shorter in profile than it should, either that the tip does not project out far enough or that the dorsum just above the tip appears to be too high. As a result, rather than being smooth and graceful, the tip seems rounded and blunted. It also lacks supratip break. Supratip break is visible in the last image of the schematic below, when the tip extends slightly beyond the dorsum. As a result, a small depression appears immediately above the tip. This is a really desirable feminine nose feature.

    Furthermore, the columella (the skin bridge between the nostrils) appears to be dangling excessively low. On the frontal aspect of the nose, the infratip lobule (the area right below the tip of the nose) is frequently still prominent and drooping. The most prevalent cause is that surgeons who utilize the closed method rarely address the relationships in the bottom third (tip, infratip lobule, ala, and columella). The hump on the nose is simply removed, leaving the rest of the nose unaffected.

    Because it is extremely difficult to treat the interrelation of these structures (of the bottom part of the nose) using a closed approach, they are all disregarded together, making the procedure easier and faster for the surgeon. Even when the open approach is used and complete access to the lower lateral cartilages of the nose (which are responsible for the tip, infratip lobule, ala, and columella’s outer appearance) is obtained, many surgeons find it difficult to master a technique that will result in ideal tip aesthetics at the end of the procedure. As a result, using a closed technique confronts the surgeon with an even larger hurdle.

    Rhinoplasty® FAQ

    After rhinoplasty, you must stay in Istanbul for 7 days. The splints placed after the surgery and the silicone pads placed inside the nose are removed on the seventh day. Therefore, you can return to your home country after the seventh day.

    On average, if you don't want anyone to notice whether you have undergone a surgery, you can return to work on the 10th day, or 7 days after the surgery if you have no concerns about it. To prevent edema, a special tape is applied on the nose after the seventh day. This is removed on the tenth day. Therefore, it is possible to start work after 7 to 10 days.
